It is common knowledge that success in guide wire crossing is the key to overall success of the PCI procedure. In the recent 10 years, the performance of guide wires have remarkably improved, and various methods of guide wire manipulation have been introduced and embraced for clinical practice.

However, the overall success rate of CTO-PCI has been remaining flat recently following the wide spread of Retrograde approach, and there has been no remarkable economic improvement such as reduction in the operation time or the number of devices used. The gap between the technical and logical progress of CTO-PCI and improvement in its clinical result is yet to be filled.

Until now, operators have been required the substantial practical experience and knowledge to operate CTO-PCI since its dawning. There is no doubt that CTO-PCI operation is the culmination of not only guide wire manipulation, but also for the overall PCI technique and knowledge, which is the reason many operators are interested in this field. Therefore, it is crucial that we overcome the technical challenges by building up a logic that offers high reproducibility in the coming years.

In this special program, we will present a new method of guide wire manipulation and discuss its effects and its remaining problems as of the present through the live case transmission following the luncheon seminar. In addition, we would like to report the latest trend of CTO-PCI worldwide.
