Group Registration (Company only)

Pre-registration Period: Wednesday, June 1 - Tuesday, September 6, 2016

If you register in ten or more people, please filling in the form and send it by e-mail.

Group Registration is now closed.

Cancellation after submitting application form, even a part of submission, will not be accepted.
Changing a registrant's name will be charged 5,400JPY(incl. tax) per each registrant.
After September 30, cancellation and any change of registrant will not be accepted.

Registration Fee

On-site registration
Medical	3 days	35,000 JPY
Medical	1 day	20,000 JPY
Medical(Intern)^*	3 days	10,000 JPY
Co-medical	3 days	4,000 JPY
Industrial Professional	3 days	35,000 JPY
* Medical(Intern) is required to present certification showing they are currently in the internship (4 years period) after graduation from medical school. Failure to do this will be charged the on-site registration fee of Medical. Pre-registration is not required. Medical Intern Certificate(English) Medical Intern Certificate(Japanese)
** A student can attend the live course free of charge. Please present your student ID or equivalent documents at the on-site registration desk. Pre-registration is not required.
